I represent and warrant to PHAT BUDDHA TATTOOS that all information is true and correct. Being of sound mind and body, I hereby reliease any and all persons representing PHAT BUDDHA TATTOOS from all responsibility. I accept any and all responsibility for any consequences that might stem from my decision to have a tattoo done by my designated artist at PHAT BUDDHA TATTOOS.
